Added Sugar Versus Intrinsic Sugar: What the Guidance Actually Says

WHO guidance targets free sugars, not all sugar. The distinction changes how a bowl of cereal or a smoothie should be judged.

A whole apple and a glass of apple juice can carry a similar amount of sugar on paper. Nutrition guidance treats them very differently, and the distinction is not pedantry. It is the entire basis of how the World Health Organization decided to write its sugar guidance in the first place.

The category that matters in most official guidance is not "sugar" in general. It is "free sugars," a term the WHO defines to include monosaccharides and disaccharides added by manufacturers, cooks or consumers, plus sugars naturally present in honey, syrups, fruit juice and fruit juice concentrates, according to WHO's own guidance page on free sugars and noncommunicable disease risk. Sugar that is intact inside the structure of whole fruit and vegetables is deliberately excluded from that definition, because it arrives bundled with fibre, water and micronutrients that change how the body absorbs it.

What the WHO guideline actually recommends

The headline figure is a ceiling: WHO recommends that free sugars make up less than 10% of total daily energy intake for both adults and children, with a further conditional recommendation to bring that down below 5% for additional benefit, according to summaries of the official WHO guideline. For an adult eating roughly 2,000 calories a day, the 10% threshold works out to about 50 grams, or 12 teaspoons, of free sugar. The 5% aspirational level is closer to 25 grams, or six teaspoons, which is a genuinely tight target once a single sweetened drink or flavoured yoghurt is factored in.

Why juice and honey sit inside the limit, and whole fruit does not

This is the part people find counterintuitive. A glass of orange juice is nutritionally closer, sugar-wise, to a sweetened soft drink than to an orange, because pressing and straining fruit removes the fibre matrix that slows sugar absorption in the gut. WHO's definition of free sugars is explicitly broader than "added sugars" for exactly this reason: it captures juice and honey even when nobody has technically "added" anything, because the sugar is now free to be absorbed quickly rather than locked inside plant cell walls.

Where the science is still being argued

It would be dishonest to present the 10% and 5% thresholds as settled beyond debate. A 2024 paper in npj Science of Food argues that the quantitative sugar guidelines deserve reconsideration, and earlier analysis published in the peer-reviewed literature has questioned whether the evidence base for the specific 10% cutoff is as strong as its confident phrasing suggests. This is a genuine, active scientific argument rather than fringe pushback: the disagreement is over how strong the evidence is for the exact numerical thresholds, not over whether free sugars in excess are harmless. Even critics of the specific percentage generally agree that current average intakes in most high-income countries sit well above whatever the right number turns out to be.

What this looks like at breakfast

Breakfast is where the free sugars debate becomes concrete fastest, because breakfast is where cereal, juice and flavoured yoghurt tend to cluster in the same fifteen minutes. Regulatory standards used in institutional child feeding programmes cap cereal sugar content at 6 grams per dry ounce, roughly 21 grams per 100 grams of dry cereal, a threshold set specifically because so many mainstream children's cereals exceed it. Advocacy research from the Environmental Working Group has repeatedly found popular children's cereals carrying 12 grams or more of added sugar in a single labelled serving, which by itself can consume roughly a quarter of a child's 10% ceiling before the school day has started, given that children eat proportionally fewer total calories than adults.

A glass of orange juice alongside that bowl adds close to another 20 grams of free sugar under the WHO definition, even though nothing on the carton says "added sugar," because the juice itself counts. None of this means cereal or juice cannot fit into a day. It means the arithmetic runs faster than the packaging suggests, and a breakfast that looks entirely virtuous, cereal, juice, a spoonful of honey in tea, can quietly use up most of a day's free sugar allowance before lunch.

Simple swaps that respect the same evidence

  • Choose whole fruit over juice most days; the fibre that comes with it is not a footnote, it changes the metabolic response to the same sugar.
  • Check cereal labels against the 5 to 22.5 gram per 100g sugar bands used in most traffic light systems rather than trusting front-of-pack claims.
  • Treat honey and syrup as free sugars in the kitchen, not as a virtuous substitute for table sugar, since WHO's definition places them in the same category.
  • Build breakfast around protein and fibre (eggs, oats, plain yoghurt with fruit) to reduce reliance on sweetened cereal or juice as the default.

This article summarises public health guidance and ongoing scientific debate; it is not individual dietary advice. People managing diabetes or another condition affected by sugar intake should follow guidance from their own clinician or dietitian rather than a general population threshold, since individual targets can differ meaningfully from the population-level recommendation.

The apple and the apple juice were never nutritionally equivalent, whatever the label on the carton implies. The guidance is not asking anyone to fear sugar in general. It is asking for attention to where the sugar came from and what, if anything, came along with it.
