A Bedroom That Is Not Quite Dark Is Linked to a Thicker, Stiffer Heart, in the First Study to Measure Both

Eleven thousand people wore a light sensor for a week. Three years later, those who slept in more than three lux, a very dim room, had thicker left ventricles and muscle that flexed less. The differences are small. Cardiologists still care.

Three lux is not much light. It is a bedroom with the curtains half drawn against a street lamp, or a clock radio and the standby lights on a television. It is dim enough that you would call the room dark, and bright enough that, with your eyes closed, you could tell it was not. A study published this week in the European Heart Journal finds that people who slept in that much light or more had measurably different hearts a few years later than people who slept in almost none.

What was done

The work comes from Professor Lu Qi's group at Tulane University in New Orleans, using UK Biobank. Some 11,071 participants wore a light sensor on the wrist for seven days, which is how the researchers know what their nights actually looked like rather than what they remembered. About three years later the same people had cardiac MRI scans. The comparison was between those exposed to more than three lux overnight and those exposed to almost no light at all.

The high-exposure group had thicker walls in the left ventricle, the chamber that pumps blood to the body, which left less room inside it. Their heart muscle also flexed less during each beat, a measure cardiologists call strain and treat as one of the earliest signs that a heart is starting to work less well. Smaller changes showed up in the right ventricle and the left atrium. "This is the first study of its kind, and it shows that exposure to higher levels of light at nighttime is linked to cardiac remodelling," Qi said. "This is where the structure and function of the heart changes, and we typically see it in response to chronic stress or injury to the heart."

How big the differences are

Small. NBC's report puts the differences at a couple of per cent in wall thickness and in contraction, which is what a population signal usually looks like: nothing any one person would notice, and a shift across eleven thousand people that is hard to explain away. The study is observational. It shows that people who sleep in light have slightly different hearts, not that the light did it, and people who sleep with the lights on may differ in other ways, from shift work to the streets they can afford to live on. Qi's group adjusted for the usual suspects, but a wrist sensor cannot rule out everything.

What makes cardiologists take it seriously is that it fits. An earlier analysis of more than 88,000 UK Biobank participants with the same sensors, posted last year on medRxiv, found higher rates of heart failure, heart attack and stroke among people with the brightest nights over the following years. This week's paper supplies a plausible mechanism for that: the heart of a person who sleeps in light is already changing shape before anything goes wrong. And there is a well-understood pathway. Light at night is the strongest signal the body's clock receives, and the clock, as Northwestern University's Kristen Knutson put it, "controls 24-hour patterns" in sleep, hormones, blood pressure and inflammation. Disturb the clock every night for years and it would be surprising if the heart were unaffected.

What the editorial says

The journal ran the paper with an editorial by Professor Thomas Münzel of the University Medical Center Mainz, who has spent much of his career on noise and air pollution as cardiovascular risks and now adds a third environmental exposure to the list. "Light pollution has emerged as a new risk factor for cardiovascular disease," Qi said, and Münzel went further: darkness, he wrote, "deserves recognition as a vital sign, as essential to cardiovascular health as blood pressure control and clean air." His advice to clinicians is to ask about the sleep environment, and his advice to patients is unglamorous. Blackout curtains. A warm-coloured, dim bedside light rather than a white one. Tape over the standby lights on the router and the television. Street lighting that dims or switches on motion, which is a matter for councils rather than households.

What we make of it

None of this is a reason to lie awake worrying about the glow of a phone charger. It is a reason to treat the darkness of a bedroom as something worth arranging, in the same category as the temperature and the mattress, and to be sceptical of the idea that a light left on does no harm because you are asleep and cannot see it. We argued in our piece on sleep hygiene that most bedtime advice is about comfort rather than health. This is one of the few pieces of it with a heart scan behind it. In the Gulf, where summer nights are lit and air-conditioned and city light is everywhere, the cheapest intervention in cardiology may turn out to be a heavier curtain.
